We always strive to offer a high quality service that has an impact on the lives of everyone who comes here. It is really important to us that we hear your feedback so that we can reflect and make plans for how to do things better.
The feedback you provide is saved on your (or your child’s) confidential individual file and will also be shared with your therapist(s). We include your feedback anonymously in a whole-service outcomes report which we create twice a year.
Ofsted is particularly interested to read our anonymised feedback from children, families and young adults who are adopted and they require us to collect it.
Once you have downloaded and completed the relevant form below, please return it to us at firstname.lastname@example.org. If you tick the box to say that you would like a manager to respond to your feedback then we will be back in touch with you within two weeks.
If you are a parent/carer of a young person who has been supported by us, please do help them to fill out a questionnaire too so that we can hear all about what they think.
Paper copies are available from reception or you can ask us to post one to you. You are also welcome to call us on 01444 413 939 to give us feedback over the phone.
